Physical Therapist (PT) – Home Health

Contract • Part-Time • PRN
$80–$100 per visit

Divine Home Nursing is seeking a skilled and compassionate Physical Therapist to provide high-quality, patient-centered therapy services in the home setting. This role focuses on restoring function, improving mobility, relieving pain, and preventing long-term disability for patients recovering from illness, injury, or chronic conditions.

The ideal candidate is energetic, clinically confident, and an excellent communicator who thrives in a flexible, autonomous environment.

Key Responsibilities

• Provide direct patient care, including evaluation of musculoskeletal, neurological, and functional impairments.
• Conduct initial and ongoing assessments to determine patient progress and therapy needs.
• Review and explain physician orders to patients and families.
• Communicate patient updates and collaborate with the interdisciplinary care team.
• Deliver high-quality, evidence-based therapy in accordance with the plan of care.
• Travel to patients’ homes to provide services.

Required Qualifications

• Licensed Physical Therapist (PT) through the Georgia State Board of Physical Therapy.
• Ability to travel to patient homes.
• Bachelor’s, Master’s, or Doctorate in Physical Therapy.
• Physical Therapy License (Required).
• CPR Certification (Preferred).
• PALS Certification (Desired).
• COVID-19 Vaccination (Required).
